Awards criteria

ACTIVITY PROFESSIONAL OF THE YEAR

  1. Any healthcare professional may nominate any other member and submit the name to the Awards Chairperson.
  2. The deadline for submitting the nomination form and supplemental letters must be no later than two months prior to “Annual Conference”. All information must be submitted in one package.
  3. Activity Professional of the Year nominees must meet the following criteria:
    a) Active CAPA membership in good standing for a minimum of 1 year.
    b) A letter from the Nominator stating why the Professional should receive the award. Including in the letter should be highlights of their contributions, dedication, service, leadership, and accomplishments.
    c) Letters of support from the following: at least one Resident or Resident/family member and one coworker [other than the Nominator].

ADMINISTRATOR OF THE YEAR

This award will be presented to an administrator who has consistently given support to activities in a facility where a CAPA member is the activity professional and who has:

  1. Supported the promotion of a better quality of life for the residents.
  2. Encouraged CAPA involvement of the activity professional.
  3. Supported continuing education and the professionalism of the activity professional in his/her facility.
  4. Demonstrated an understanding of the value of activities in the improvement of the quality of life for residents.

OUR PACK AWARD

  1. Any member may nominate any other member and submit the name to the Awards Chairperson.
  2. The deadline for submitting the nomination form and supplemental letters must be no later than two months prior to “Annual Conference”. All information must be submitted in one package.
  3. The ‘Our Pack’ nominees must meet the following criteria: a. Active CAPA membership in good standing for a minimum of 10 years. b. Outstanding contributions in the field of activities as evidenced by letters of recommendation.
  4. Nominator must provide the following supportive documentation:
    a) 2 to 4 letters of recommendation from fellow professionals, employers, clients, community leaders, etc.
    b) 2 to 4 letters from persons who feel the nominee gave them support and/or inspiration in their profession.
    c) A letter from the nominator and a completed nomination form.
  5. The ‘Our Pack’ Award recipient will receive the award as determined by the State Executive Board and recognized at Annual CAPA Conference and in CAPA Publications.
  6. The ‘Our Pack’ Award will be awarded to a nominee once in their lifetime.
  7. Nominators will be notified of the nominee’s status prior to the state conference registration deadline date of the appropriate year.
